Can Cannabinoids Help Reduce Agitation in Alzheimer’s Disease?

Over the past decade, researchers have explored how cannabinoids—chemical compounds in the cannabis plant—impact health. One area of focus is whether these compounds can reduce agitation, a common behavioral change in people with Alzheimer’s disease.

The Challenge of Agitation in Alzheimer’s Disease

Agitation is a frequent and distressing symptom in Alzheimer’s disease. Non-drug approaches are the preferred first line of treatment, but medications are often needed for severe cases that don’t respond to other methods. Current medications can be effective, but they often come with significant risks, including stroke and even death.

Cannabinoids may offer a safer alternative. These compounds interact with the body in a unique way, potentially reducing agitation without the severe side effects associated with traditional drugs. Some cannabinoids may even protect brain cells from damage.

Why More Research is Needed

Despite their promise, there is limited research on cannabinoids as a treatment for agitation in Alzheimer’s disease. Cannabinoids may affect brains with Alzheimer’s differently than healthy brains. For example, while cannabinoids are known to have calming effects in the general population, they could produce different or even opposite effects in those with Alzheimer’s.

To explore this further, our team at Sunnybrook Research Institute launched a pilot study with support from the Alzheimer’s Drug Discovery Foundation and the Alzheimer Society Research Program.

A Pilot Study on Nabilone

We conducted a 14-week trial with adults who have moderate to severe Alzheimer’s disease. Participants received nabilone, a synthetic cannabinoid approved in Canada for chemotherapy-induced nausea, to treat agitation and aggression. Using a placebo for comparison, we aimed to determine whether nabilone could reduce agitation without causing serious side effects.

We also investigated how nabilone might address pain and weight loss, two common issues linked to agitation in advanced Alzheimer’s. Cannabinoids are associated with improved pain relief and appetite, so we measured changes in these symptoms as well.

Promising Results

The study found that nabilone significantly reduced agitation compared to the placebo. Participants also experienced overall improvements in behavioral symptoms. The most common side effect was increased sedation. These results are encouraging, but further research is necessary. Larger trials will help confirm the safety and effectiveness of nabilone for this purpose.

Moving Forward with Caution

While our findings highlight the potential of cannabinoids in treating Alzheimer’s symptoms, it’s important to proceed cautiously. Physicians, patients, and families should avoid using cannabinoids for agitation until more research is conducted. Future studies must replicate these results and closely examine potential side effects, including sedation and memory impairment.

The Importance of Further Research

Continued research is essential to understanding the benefits and risks of cannabinoids for Alzheimer’s disease. With more data, we can develop evidence-based guidelines and policies that prioritize the well-being of individuals living with dementia. Instead of short-term fixes, this research will help us focus on improving mental health and quality of life for people with Alzheimer’s disease and their families.
